This one hour workshop will walk you through advanced directives including Health Care Proxy and MOLST (DNR/DNI) paperwork. Registered nurse Nancy Aureli, will give you the forms, provide examples on starting the conversation, and how to choose a health care agent. There will also be a question and answer session at the end.
