The Chronic Disease Self-Management Program (CDSMP), is a six session peer led health education program for adults (18+) with any type of ongoing health condition. Each session complements the health care you may already be receiving. The purpose of the CDSMP is to enhance your ability to manage your health and maintain an active and fulfilling lifestyle. You do not need to have a chronic illness to attend; maybe you know or care for someone who does. All class participants will receive a FREE ($18.95 value) Living a Healthy Life with Chronic Conditions book. CDSMP workshops will help you • Feel Better. Increase your energy and get relief from pain, fatigue and difficult emotions. • Take control of your life. Do the things you want to do each day. • Get connected. CDSMP is an evidence based program. The curriculum was developed at Stanford University's Patient Education Center and is brought to you through a collaboration among Onondaga County Department of Adult and Long Term Care Services, The Advocates for Upstate, and OASIS, a program of Upstate Medical University.
